New Clients
How to be prepared for your visit:
- Bring your driver’s license.
- If you are a new client, please bring in your previous records or have them faxed to 816-436-4030 or email them to [email protected]
- Please arrive fifteen minutes prior to your appointment to fill out paperwork.
- If you plan on running a fecal test, please bring your fresh sample.
